Community EMS: Bridging the Gap

Campbell County Health Emergency Medical Services is breaking ground on a new service line to alleviate a healthcare gap within the community.   

CCH now offers Community EMS Care Management. This program aims to increase access to healthcare resources for patients in Campbell County.  The program allows Community EMS Providers to visit a patient’s home, evaluate their individual needs and in some cases, provide or assist with aftercare following a recent discharge from the hospital.   

Community EMS can also expedite communication between care providers ensuring that all providers are updated on a patient’s condition or progress when necessary.   

Currently, Community EMS Care management services are only available to Medicare B participants and Medicaid recipients.   

To date, Community EMS has: 

  • 58 patients in the program 

  • 13,742 patient contact minutes 

  • 5 EMS staff doing home visits 

With plans to expand once the service is well established, Community EMS brings our community a lifetime of care with dedication, skill and compassion, all in the comfort of your home.
