CCH has been providing onsite face-to-face health coaching services since 2006. We have found the most success using year-round health coaching and motivational interviewing. This process focuses on the participant and coaching to the individual, not pre-established goals. Those self-directed goals might include weight loss, smoking cessation and/or new healthy lifestyle routines. Health Coaches build rapport with the participant, using active listening skills and eliciting behavior changes.
When an employee suffers an injury at work, it can drastically affect your company. With the best interests of the employee and employer in mind, our trained case management staff will help guide your employees through the injury care process. If necessary, case management will help the employee coordinate with referring specialists and communicate frequently with the employer about this process. Case Management is designed to provide a streamlined process for the injured employee, employer and healthcare providers.
