Home Health and Hospice Partnership with Legacy Living and Rehabilitation

Providing hospice care to patients in northwest Wyoming requires collaboration and teamwork. As part of our video series highlighting the important work of home care and hospice staff at CCH, we also wanted to share the hospice work that takes place every day at the Legacy Living and Rehabilitation Center for patients aging in place.

Legacy Living and Rehabilitation offers both long-term care and short-term rehabilitation. Committed to providing dignified care and purposeful living, The Legacy was built with the residents and their families in mind. Part of long-term care is aging in place and supporting Legacy residents in their final phase of life. This is the story of one family.
