November is National Home Care and Hospice Month

It takes an incredible person to be able to take on the care of another, especially toward the end of their life. This November, CCH wants to honor our nurses, home care aides, therapists, and social workers who make a remarkable difference for the patients and families they serve.

Ninety percent of Americans want to age in peace at home. Home Health is the preferred method of healthcare delivery among the disabled, elderly, and chronically ill. Home care provides high-quality, compassionate care to more than 5 million Americans annually.

In addition to providing amazing, quality home care, CCH offers Home Health Telemonitoring. The first service of its kind offered at Campbell County Health in Gillette, Wyoming, telemonitoring transmits clinical information like blood pressure from the patient’s home to trained home health nurses in about four minutes. If the home health nurse determines that a patient could benefit from home monitoring, patients receive a kit that contains a scale, blood pressure cuff, pulse oximeter (to measure oxygen levels in the body) and a transmitter. Additional equipment, like a glucometer (a device that measures blood sugar levels) can be added to the kits. The patient is then trained to use the equipment until they feel comfortable doing it without a Home Health nurse.

CCH Hospice provides comfort care and symptom/pain management for anyone diagnosed with a life-limiting illness. Care is initiated by the patient, family or physician and provided day or night where the patient calls home.
