Governor Recognizes Community EMS and Chronic Care Management Programs with MIH Week Proclamation

GILLETTE, Wyo. - Oct. 24, 2024 - Earlier this week, representatives from CCH EMS, including Manager Shane Kirsch, Paramedic Shift Supervisor Jason Frye, and EMS Chronic Care Manager Danielle Wood, traveled to Cheyenne to join Governor Mark Gordon for the signing of a proclamation recognizing October 6-12 as Mobile Integrated Health (MIH) Week in Wyoming. CCH’s MIH programs include Community EMS and Chronic Care Management, offering mobile solutions that are transforming access to care for rural communities in Northeast Wyoming.

MIH is a patient-centered model of care that extends the role of EMS providers beyond emergency response to offer proactive, in-home healthcare services. This approach fills critical gaps in access to care, especially in rural areas, by deploying EMS professionals to provide non-emergent, scheduled visits directly in patients' homes.

These visits may involve health assessments, medication management, chronic disease monitoring, fall prevention, wound care, and other services. MIH is designed to reduce the strain on emergency departments and clinics by preventing unnecessary hospital admissions, avoiding 911 calls for non-urgent needs, and improving continuity of care for patients with chronic conditions.

The program also helps integrate EMS professionals more deeply into the healthcare ecosystem, acting as a bridge between patients and primary care providers, social services, and specialists. With a focus on improving patient outcomes and easing the overall burden on healthcare systems, MIH is quickly becoming a critical component of rural healthcare delivery.

“These programs are vital,” said Jason Frye, who leads CCH’s Community EMS and Chronic Care Management programs. “They fill healthcare gaps in rural communities, reduce ER visits, admission rates, and offers patients the care they need in their own home.”

The personalized care provided by these programs also enhances the patient experience by building trust and fostering long-term relationships. By leveraging EMS personnel for proactive, community-based care, CCH provides cost-effective solutions to the challenges rural communities like ours face when seeking care.
